This is the last tape I have.  It is labeled:

I never used it because I don't think I had the docs and I never had a 
problem sticky enough to need it.  One of the versions on the tape was never 
able to load but I was able to create it by comparing the differences between 
two others and making the necessary changes to create the bad one.
